Alumnus of the Year Awards

Alumnus of the Years Awards will be presented at the 2008 Honors Convocation of the College of Health Professions.

Professional disciplines include:

Health Services/Administration
Health Systems Management
Nurse Anesthesia
Nursing
Physician Assistant
Allied Health (Nutrition, Medical Records, Medical Technology, Health Care Education)

If you know of an outstanding alumnus who deserves recognition, please complete this nomination form and return it to the address below by January 30, 2008.

Prior to nominating an individual, it is important to ascertain that the honoree will be able to attend the Honors Convocation on Sunday, April 6, 2008 during which the Alumnus of the Year will be presented.
